Silent Hill: Townfall is set to feature five distinct endings, mirroring the approach taken by previous installments in the long-running horror franchise. This tradition of multiple conclusions has become a hallmark of Konami's Silent Hill series, with Silent Hill f also offering five endings in 2025, one of which was noted for its comedic tone. Last year's Silent Hill 2 remake presented players with eight different endings, five of which were unlockable only after a subsequent playthrough.

During a preview event, writer and director Jon McKellan shed light on how player choices will influence the game's outcome. While not directly confirming the existence of alternate story paths, McKellan described the game's structure as "wide linear," allowing for some deviation and player agency. He indicated that the order in which players tackle certain sections could vary, providing a less on-rails experience.

McKellan clarified that the factors contributing to the game's multiple endings are subtle, avoiding overt A/B decision points. Instead, the game monitors how the player embodies the protagonist, Simon, through their actions and behaviors. "Everything counts," McKellan stated, suggesting that the cumulative impact of player choices, however understated, will shape the narrative's conclusion and reflect the type of player they are. The game is scheduled for release on PlayStation 5 and Windows PC on September 24.
