The mission begins with investigating the subway system within the Evacuation Zone, specifically Railyard Street Station. Players should start at the 'Main Street' fast travel point and proceed north towards a construction site infested with Mold. Clearing this Mold wall reveals the path to the subway station. Inside, players will find resource chests and must navigate tunnels, avoiding trains and utilizing the 'Shift' ability to traverse different levels. After defeating Mold monsters and traversing further tunnels, players will eventually discover the Fault itself, marked by an interactable TV. Completion requires having first finished 'The Subway Fault' to possess the 'Reach' ability.
The Fault is located beneath an island of orange pipes in the Underpass. Players must reach this island, defeat any Mold, and then drop down to a ledge to access the Fault's interior. After proceeding through a corridor, players interact with a TV to enter the Fault.
Inside, the core objective becomes guiding light-projected children. A yellow aura around Dylan's head indicates the children are following. Players must follow a specific path, including a left-hand path onto a tilted platform, jumping between platforms until reaching two large shadows. After interacting with a wall, the next objective, 'Reunite young Dylan and Jesse,' begins.
To reunite young Dylan and Jesse, players must follow a series of platforming and traversal challenges. This involves navigating through shadow rectangles, jumping between platforms, and taking specific paths. A key challenge is locating and guiding the light-projected Jesse.
