Running a hospital in Roblox's horror game Animal Hospital involves more than just treating patients; it requires vigilance against the titular Anomalies, entities that pose a threat to both the player and the hospital's inhabitants. These creatures can be disguised as patients or manifest as environmental hazards, making identification a crucial first step in survival. This guide details all known Anomalies and the strategies required to overcome them.

Patient Anomalies

These Anomalies attempt to blend in with regular patients at the reception desk. Players must be observant for subtle and not-so-subtle visual cues.

  • Twitching: A patient exhibiting rapid, unnatural twitches signals an Anomaly. The recommended action is to close the shutters.
  • Three eyes: A patient with three glowing eyes is an Anomaly. Close the shutters to mitigate the threat.
  • Sharp teeth: This Anomaly presents with a menacing smile revealing sharp teeth and has lopsided, googly eyes. Closing the shutters is advised.
  • Clenched teeth: Similar to the sharp-toothed Anomaly, this patient displays human-like clenched teeth and wide-open, small eyes. Close the shutters.
  • Sinister smile: Characterized by a sinister smile and large, wide-open eyes that appear hand-drawn, this Anomaly also requires closing the shutters.
  • Hollow face: Patients with hollowed eyes and a blank, jet-black expression indicate an Anomaly. Close the shutters.
  • Stalker check-in: The Stalker, a creature that sometimes lurks in the hospital, may attempt to check in at reception. Taking its picture is the recommended response.

CCTV Anomalies

After the first shift, players gain access to a security camera system, which reveals Anomalies that appear normal on camera. These include:

  • Unnatural body: While appearing normal at reception, the CCTV feed shows this Anomaly with unnaturally stretched limbs. Close the shutters.
  • Camera watcher: This patient seems ordinary at the counter but stares directly at the security camera when viewed on CCTV. Close the shutters.
  • Shadow: On CCTV, this Anomaly appears as a completely dark silhouette, despite looking normal at reception. Close the shutters.
  • Redacted box: A black, censored-style box with a set of eyes appears over the character on security camera footage. Close the shutters.
  • Skinwalker: These monsters, known for sharp teeth and wide mouths, appear normal at reception but are revealed as Skinwalkers on CCTV. Close the shutters.

Photo Anomalies

Polaroid photos taken during check-in can reveal Anomalies through static, mismatched features, or distorted images.

  • Static: If a patient's photo is covered in static, it signifies an Anomaly. Turn the character away.
  • Wrong eyes or ears: Anomalies can be identified if a patient's photo shows eyes or ears that do not match the actual patient. Turn the character away.
  • Mismatching polaroid: If the polaroid displays a different character model or a grotesque expression on a similar model, it's an Anomaly. Turn the character away.
  • Cursed photo: A blurred photo with a grotesque expression indicates an Anomaly. While picking it up can cost Sanity, it can be identified on the desk before collection. Turn the character away.

Hazard Anomalies

These Anomalies can spawn within the hospital regardless of patient screening, requiring immediate attention or avoidance.

  • Mass of eyes: A grotesque blob of flesh and eyes attached to a ceiling. Players can offer eye drops or avoid eye contact for it to disappear.
  • Hiders: Transparent characters camouflaged except for their eyes and smile. Running back to the lobby or using a Taser or Gun will eliminate them.
  • Skinwalker: If an Anomaly is admitted, it can transform into a Skinwalker. A Taser, Gun, or bare hands can be used for defense.
  • Bed Monster: Large hands protrude from under a patient's bed. Players must avoid the large red area indicating its presence.
  • Stalker: This enemy, known for scrawny limbs, attempts to surprise players. Avoiding eye contact prevents Sanity loss.

  • Camera Anomaly: A cat-like enemy appearing when using broken security cameras. Quickly pressing the 'Exit' button on the camera screen is necessary to avoid Sanity loss.
  • Head Banger: A character with hollow eyes and a void-like mouth banging its head against reception windows. Offering a cup of coffee may cause it to leave.
  • Tendril: Purple tentacles spawning from a recovering patient. Successful surgery can save the patient.
  • Slime: Green slime on the floor can be cleared with a fire extinguisher.
  • Fire: Empty rooms may catch fire, extinguishable with a fire extinguisher.
  • Ghosts: Haunting the hospital when patients are not saved. A Taser or Gun can be used.
  • Fake patient: Skinwalkers disguised for emergency surgery. A Taser is required.
  • Death rituals: Sinister rituals involving levitating patients and candles. Use coffee to extinguish the candles.
