Anomalies are the primary antagonists in the Roblox horror game Animal Hospital, presenting a constant threat to players managing the facility. While some patients seek legitimate medical care, others are intent on causing harm. Recognizing these disguised threats is crucial for survival and successful hospital management.

Players can encounter dozens of Anomalies, which can be broadly categorized into four types: Patients, CCTV, Photos, and Hazards. Each category presents unique challenges and requires specific methods for identification and neutralization.

Patient Anomalies

These Anomalies often reveal themselves during the initial patient check-in process at the reception desk. Subtle cues can indicate a patient is not what they seem.

A 'Twitching' patient might appear normal but will periodically twitch in an unnatural, rapid manner. The recommended action is to close the shutters. Patients with 'Three eyes' or a 'Sinister smile' with large, wide-open eyes, often resembling a hand-drawn illustration, also require the shutters to be closed. A 'Sharp teeth' patient, characterized by a menacing smile revealing sharp teeth and lopsided eyes, also warrants closing the shutters. Similarly, 'Clenched teeth' patients display human-like teeth and small, wide-open eyes. 'Hollow face' patients have blank expressions with jet-black, hollowed eyes and mouths. The 'Stalker check-in' Anomaly, which is the same enemy that sometimes roams the hospital, can attempt to check in at reception; players should take its picture.

CCTV Anomalies

Upon unlocking the security camera system after the first shift, players gain an essential tool for detecting Anomalies that may appear normal in person. These threats are only visible through the security cameras.

A 'Unnatural body' Anomaly shows no outward signs of distress but appears with unnaturally stretched limbs on camera. 'Camera watcher' patients stare directly at the security camera, despite appearing normal at the counter. The 'Shadow' Anomaly appears as a completely dark silhouette on the security feed. A 'Redacted box' Anomaly will display a black, censored-style box over the character on camera, containing a set of eyes. 'Skinwalker' Anomalies, which are otherwise normal at the counter, are revealed to have sharp teeth and wide, open mouths on the security cameras.

Photo Anomalies

Taking polaroid photos of patients during the check-in process can also reveal hidden Anomalies.

If a patient's photo comes out covered in 'Static', it indicates an Anomaly. Subtle discrepancies, such as 'Wrong eyes or ears' that do not match the patient, also signal a threat. A 'Mismatching polaroid' may show a completely different character model or a similar model with a grotesque expression. 'Cursed photo' Anomalies appear blurred with a grotesque expression, and interacting with them can reduce Sanity.

Hazard Anomalies

Despite careful screening, certain Anomalies can spawn within the hospital, requiring immediate attention to mitigate their effects.

A 'Mass of eyes' is a grotesque cluster of eyes and flesh that attaches to ceilings. Players can use eye drops or avoid direct eye contact to make it disappear. 'Hiders' are transparent characters camouflaged within the environment, visible only by their eyes and smile; running back to the lobby or using a Taser or Gun can deal with them. Skinwalkers can transform from other Anomalies within the hospital and can be subdued with a Taser, Gun, or by hand. The 'Bed Monster' hides beneath a patient's bed, indicated by its large hands and a surrounding red area; players must simply maneuver around the designated zone. The 'Stalker' enemy, characterized by long, scabby limbs, tries to surprise players but can be evaded by not looking at it, which also preserves Sanity. The 'Camera Anomaly', a cat-like enemy, appears on broken security cameras and can be avoided by quickly pressing the 'Exit' button. A 'Head Banger' with hollow eyes and a void-like mouth bangs its head against reception windows; offering it a cup of coffee may make it leave. 'Tendril' Anomalies are purple tentacles spawning from recovering patients, which can be dealt with by performing surgery successfully. 'Slime' on the floor can be cleared with a fire extinguisher. Fires in empty rooms can also be extinguished using a fire extinguisher. 'Ghosts' that haunt the hospital can be dispersed with a Taser or Gun. 'Fake patients' are actually Skinwalkers disguised for emergency surgery and can be identified with a Taser. 'Death rituals' involve levitating patients surrounded by candles; these can be stopped by using coffee to extinguish the flames.
